Your label must include
Required statement
“Products are not inspected by the Health Department”
All label information must be at least 10-point font. Statute requires ‘a statement that the food is not inspected by the department or local health department’ (no fixed wording); the quoted text is the wording on ADPH’s review form (rev. 6/7/24). Allergen disclaimer wording is not fixed by statute (e.g., ‘This food may contain allergens’). P.O. Box may be used instead of home address. QR codes cannot replace required info.
