“This food was prepared in a kitchen that is not subject to regulation and inspection by the regulatory authority and may contain allergens”
No font size is specified. NDA describes where the statement must be shown for each sales channel: displayed at events, given for home pickup or delivery, and on the website or in advertisements. Product name, net quantity and an allergen list are not listed by NDA for non-TCS foods, but including them is good practice.
