“This product is made in a home kitchen that is not inspected by the state or local health department.”
The statement may go on a product label OR on a consumer advisory sign at the point of sale. No font size is specified. The statute requires no name, address, ingredients or allergens, but they are recommended (and needed for shipped or online sales where there is no sign).
